Re: referrals for Medical Dermatology Conditions, including: skin check, single-lesion check, rash, abnormal skin color, change in skin texture, hair loss, nail abnormalities, hyperhidrosis, etc

Our goal is to make referrals as simple as possible.

If this is a NEW medical issue, or one that hasn’t been addressed by a medical professional, then patients can be referred by:

– Having the patient call Anew Dermatology directly and ask for an appointment

OR

– Faxing a referral with face-sheet and any relevant medical information

If the referral is for either a new-onset issue or for an issue that has been previously addressed (prior treatments or diagnostics) by a medical professional:

– Fax a referral, which includes: (1) a face-sheet with patient contact & insurance information; (2) ALL pathology and/or laboratory results related to the issue the patient is being referred for; (3) any additional information that may be helpful
